Our Travel Vaccinations clinic offers the following vaccines:
- polio (given as a combined diphtheria/tetanus/polio jab)
- typhoid
- hepatitis A
- cholera
These are provided by the NHS - there is no additional charge.
Please note:
- Our travel vaccination clinics are ad hoc subject to staffing and demand
- Please complete this form and return it to the practice at least 12 weeks prior to travel.
- The surgery will be in contact thereafter
- The form can be submitted in person, by post or via email
